정신과 영역에서 항 경련제 역할

Epiplesy의 진단으로 치료대책으로 약물을 선택해야 하는 것은 물론 FDA에 의해 인증된 것이 있다.
아마도 최소의 항경련제는 1857, Charles Locock가 사용한 Bromide일 것이다.
이어 Penobarbital, 이어 sedation이란 원치않는 부작용을 최소화한 것이 Phenitoin,
놀랍게도 오늘의 항경련제가 그들의 약효에서 오늘의 정산과 영역 다양한 분야에 적용되리라고는 예측하지 못했을 것,
당연히 작금의 정신과의사는 anticonvulsant에 대한 이해 정도가 아니라 그 적용법위를 확대하기 위한 연구,
거기에 엄청난 관심을 기울이고 있다.
'해암 뇌의학 연구소' 역시 예외가 아니다.
2004년 AAN American Academy of Neurology 와 AES American Epilepsy Society 는
Epilepsy Treatment의 Guideline을 제시하게 되었다.
Epilepsy로 새롭게 진단된 경우 우선 선택되어야 할 약제가 제공되었다.
carbamazepine, phenytoin, valproic acid/valproate semisodium, phenobarbital,
이어 새롭게 개발된 약품들이 소개되었다.
gabapentin, lamotrigine, oxcarbazepine or topiramate 등이 이었다.

Schizoprenia, Affective disorders-특히 Bipolar affective disorders는 물론
Personality disorders, 심지언 Dementiua, Chronic pain condittions 에 이르기까지 항경련제의 활용법위는 확대되었다.

그 적용법위가 입증되어가고 있다.
Underline causaltiy 와 관계 없이 정신분열병이나 약극성 장애 등에
aggression 과 cognitive impairment 등에 대한 특징적 증상 변화를 보여주고 있음은 특히 주목할 일이었다.

거의 결론에 이르고 있음은 바로 위에 열거된 항경련제의 영향이 적지 않다.
기본적으론
CBT congnitive behavior therapy 를 바탕에 둔 방법론으로 거기 새로운 약물과 replacement therapy가
가세 개선된 것이다. 이들 방법은 외래치료에도 적용될 수 있을 것이다.
Anticonvulsandts 에 관한 문제를 고찰하고 있는 중이다.
이들의 응용은 Nicotine Dependence에 대한 외래 치료에도 확산되고 있다.
여기엔
이들 약물과 neurotransmitter 와의 관계를 제외하면 빈 껍데기에 불과할 것이다.
Serotonin:Valporate & lamotrigine/ Dopamine:Valporate/ Noradrenalin:Lamotriginm/
Hypothlamic-pituitary activity:Lamotrigin/ GABAergic action:Gebapentin & pregabalin 등과 같은
공식이 적용되고 있다.
그렇다고 이같은 공식이 말 그대로 수학적 공식으로서 증상해결에 적용되고 있는 것은 아니다.
약품에 따라선 presynaptic 과 postsynaptic 에 각기 작용하는 것이 있는가 하면 동시에 영향력을 발휘하는 것도 있다.
Neurotransmitter 하나 하나가 동일한 작용만 하는 것은 아니다.
상황과 위치에 따라 역할이 같은 신경전달물질임에도 달라진다는 점을 참고할 일이다.
요약하건데 anticonvulsants에 대한 이해는 정신과의사가 알아야 할 필수이며 그 적용법위는 더욱 확대 될 것이다.
특히 의과대학생들을 염두 할 때 이 점 더욱 부각되어야 할 부분이라 간략히 소개한 것이다.
